Description
The Surgical Clinic Community Health Worker functions as part of the Trauma Team and Comprehensive Trauma Clinic program at Ronald Reagan UCLA. The primary role of surgery clinic Community Health Worker is ensuring patients receive essential assistance as they transition from trauma patient back into the community, reducing their chance of repeat trauma, addressing social determinants that contribute to traumatic injury and other poor health outcomes, and improving their quality of life. The Community Health Worker will engage in activities that promote, maintain and improve the health and wellbeing of post-trauma patients. The community health worker will work primarily in the outpatient setting through the Comprehensive Trauma Clinic, but also be expected to establish relationships with admitted trauma patients who are identified as likely to need further care through the Clinic once discharged, in order to ensure a seamless continuity of care. As part of this role, the Community Health Worker will initiate and establish connections to community-based organizations in the Los Angeles area, in order to provide services for our trauma patients and provide close shepherding through these resources. Examples include: mental health services, job readiness training or counseling, navigation of victim of crime compensation, GED preparation, legal advocacy, housing agencies, addiction treatment, among other resources.
Salary: $30.53 - $40.30 Hourly
Qualifications
Bachelor’s degree in social work from a program approved by Council on Social Work Education or equivalent combination of education and experience.
