The Supervisor Community Health is accountable for providing cultural mediation between the health care system, social services and the community. This position acts as a credible member and has a close knowledge of the ethnicity, language, socio-economic status and life experiences of the community served. This position also helps oversee people gaining access to needed services and the building of individual, community and system capacity by increasing health knowledge and self-sufficiency.
ESSENTIAL FUNCTIONS OF THE ROLE
Supervises daily activities of the program to ensure efficient operations, including facilitating team meetings as directed by the program manager.
Maintains outcomes data and dashboard to allow for program evaluation.
Interviews, selects, trains, supervises, counsels and recommends termination of assigned employees in partnership with the program manager.
Oversees assigned staff and provide coverage when staff is on leave.
Performs administrative duties related to payroll and other related responsibilities.
Investigates, resolves and documents resolution of patient or staff complaints.
Communicates with team members regarding protocols and practices related to their assigned area.
Provides insight as a subject matter experienced on strategic initiatives related to the role of the Community Health Worker.

QUALIFICATIONS
EDUCATION - H.S. Diploma/GED Equivalent
EXPERIENCE - 3 Years of Experience
CERTIFICATION/LICENSE/REGISTRATION
Community Health Worker (CHW): Certified Community Health Worker from Texas Department of State Health Services Required.
Cert Community Hlth Wkr Instr (CHW-I): Certified Community Health Worker Instructor required within 6 months of hire.
